MedsCheck

MedsCheck and Diabetes MedsChecks are structured and collaborative clinical pharmacy services that take place in the pharmacy to optimise the safe and quality use of medicine. These services involve a review of patient medicines, a face-to-face consultation between the pharmacist and patient, and the development of a medication profile and an action plan.

The benefits of having a MedsCheck consultation with a pharmacist are:
• Understanding how your medications are helpful for you and your health conditions
• Identifying any issues with how you currently use your medication and make suggestions to help you get more out of your medication
• Looking at any potential interactions between your medications

Home Medication Review (HMR)

A HMR is a consumer-focused, structured and collaborative health care service provided in your home, to optimise use of medicines and consumer understanding.

An HMR requires a referral from your GP to an accredited pharmacist.

The benefits of an HMR are:

  • Provides the patient (and carer) and your GP with a comprehensive, up-to-date summary of all the medicines, complementary products, devices and other prescriptions being used by the patient.
  • Reinforces the medication related advice given by the GP.
  • Provides recommendations for the GP on any potential or actual drug related problems that may be an issue for the patient.
  • Assists the patient with medications/aids/devices to improve compliance, early detection and management of medicine-related issues.
  • Provides patients with an understanding of their medicines and enhances their ability to manage their medicines appropriately.

Webster-Pak Medication Management

Webster-paks provide a simple way to take the worry out of medications. By having your regular medications packed into a weekly blister pack you can ensure you are getting the right dose at the right time.

A Webster-pak is ideal for people:
• On multiple medications each day
• With complicated mediation regimens
• Prone to missing or forgetting doses
• Who want a simpler, more efficient way to manage their medications

Webster-paks work well because:
• There is a visible reminder to take medication
• It is easy to see if medication has been taken or not
• All age groups can use them, as they are convenient and portable
• They reduce the amount of medicine in the home

Return Unwanted Medicines (RUM)

The RUM Project provides consumers with a free and convenient way to dispose of expired and unwanted medicines. Do you have expired or unused medications in your home, and want to know what to do with them? Anyone can return their household medicines to our pharmacy anytime, for safe collection and disposal.

  • Storage of expired and unwanted medicines in the home can be harmful
  • These can be especially hazardous to small children if they are within reach.
  • Elderly people may become confused with too many medicines in the home and this can lead to “medication mismanagement”, often requiring hospital care.
  • Medicines that are disposed of down the sink or toilet, or put in rubbish bins, can also lead to poor environmental consequences.

Prescription Record Form Safety Net Report

The PBS Safety Net protects patients and their families requiring a large number of PBS or RPBS items. You may be eligible for an extra prescription subsidy if you and your family require a large number of PBS items in a calendar year. Under this government scheme you are responsible to keep track of the cost and number of items. There are a number of rules to the process.

At Scott-Dibben Chemist, we can help you manage the process by keeping an electronic report of the cost and number of your medicines.
The scheme requires pharmacists, on request by patients, to record the supply of PBS and RPBS items on prescription record forms. When a patient reaches the Safety Net threshold within a calendar year, they qualify to receive PBS or RPBS items at a cheaper price or free of charge for the rest of that year. Any applicable special patient contributions, brand premiums or therapeutic group

For the purposes of the scheme, the family includes the person:

  • The partner or de facto partner
  • Children under the age of 16 who are in the care and control of the person
  • Dependent full-time students under the age of 25
